For immunization appointments click on the button below and you will be taken to the Scripted home page where you can easily fill out you information and select the vaccine appointment of your choosing. Please note there are 2 types of vaccines, Routine and Travel. See below to see which one to choose. Please note you will need to receive a text message to fill out the required forms.

Routine Vaccines- Shingles, Pneumonia, HPV, Tdap, Meningitis, Hepatitis A and Hepatitis B

Travel Vaccines- Yellow Fever, Meningococcal, Typhoid, Hepatitis A, Hepatitis, Polio, Cholera, Japanese Encephalitis and Rabies.
